How to get your Organizing Special Appointment

1. Choose your Special from the list below under PAY FOR SPECIALS HERE  below.

     See “Distance” information at bottom if over 40 miles from Longmont.

2. Pay by Credit/Debit card - PAY BELOW. All specials are listed with the ADD TO CART button below. Click on the drop down button below to find your choices. Then click on the ADD TO CART button.

Pay by Check: Send check payable to Dr. DeClutter, PO Box 6149, Longmont, CO 80501 - make sure contact and email information is included (see #4). Postmarked Saturday, March 6.

3. Click on the UPDATE TOTALS on the PayPal screen to see your total before completing the transaction.

4. You will hear from me by email that I have received your payment and I will send you an APPOINTMENT PREP TOOL LIST. If you haven’t already scheduled the appointment, we will by phone or email. Can’t wait!
